Onsite Occupational Health & Safety Inc., ranked No. 101, was one of six Indiana businesses to make the top 500 and one of 65 in the top 5,000.
The Inc. 5000 ranks companies by percentage of revenue growth during the previous three years.
Founded in 2008, Onsite OHS recorded about $19 million in revenue in 2011 and grew 3,007 percent over three years. Employment at the company grew from four to 165.
Indiana’s top company on last year’s list, Slingshot SEO Inc., fell to No. 136 from No. 58 a year ago.
The Indianapolis-based Internet marketing consultant, which had $10.9 million in revenue last year, grew 2,391 percent over the past three years, according to Inc. 5000.
Slingshot increased employment from 10 to 100 people between 2008 and 2011. But the company announced earlier this month it would lay off about 15 people.
The only other Indianapolis company in the top 500 was Stonegate Mortgage, at No. 478.
The mortgage lender’s revenue increased 807 percent, to $23.7 million, and employment grew from 40 to 215.
Other Indiana companies in the top 500:
— No. 176 Day Online Solutions LLC of Sellersburg grew revenue 1,949 percent, to $2.7 million;
— No. 202 Livin’ Lite Recreational Vehicles of Wakarusa grew 1,793 percent, to $8.9 million;
— No. 326 StrataShops of Elkhart grew 1,155 percent, to $3.5 million.
Livin’ Lite was the only other Indiana company besides Slingshot to return to the top 500.
