The Indianapolis-based health system said Nov. 3 that it recovered the bag within hours and began an internal investigation.
The paperwork may have contained patient names, physicians’ names, dates of birth, genders, medical records numbers, dates and times of service, diagnoses and procedures. The papers did not contain financial information or Social Security numbers.
The system said it “has no reason to believe” the information has been improperly accessed or used. The incident, which occurred Sept. 26, affected certain patients who had procedures at IU Health Ball Memorial Hospital and surrounding facilities from July 9, 2012, to April 25, 2017.•
