Cases have risen above 1,000 mark only three times in the last 34 days.
The seven-day moving average of positive cases in Indiana was 938 on Tuesday, up from 759 two weeks ago.
Indiana reported the death of just one person from COVID-19, the lowest number since March 22.
The department reported the testing of 4,618 new individuals.
Statewide hospitalizations due to COVID-19 rose from 621 on Monday to 657 on Tuesday. The high mark was 3,460, set on Nov. 30.
The state said more than 1.13 million Hoosiers had been fully vaccinated against COVID-19 as of Wednesday at 5 a.m. More than 1.65 million had received the first dose of a two-dose vaccination.
